Do I need one?

Yes, if you're pairing a concealed valve with a slide rail kit. The valve is buried in the wall and the rail kit is just rail, handset and hose — the elbow is what joins the two, providing a neat wall outlet for the hose to screw into.

When you don't need one

You can skip it if you're only fitting a fixed overhead drencher, since the drencher arm connects directly. You can also skip it if you choose the wall outlet bracket with handset and hose, which combines outlet, bracket and handset in one piece.
